The Primary Care Nurse Practitioner / Physician Assistant obtains medical histories and performs medical evaluations for our patients, treats medical problems, maintains records, and provides health education. We adhere to the Patient-Centered Health Home (PCHH) mode of care. Our mission is to achieve regional health through high-quality comprehensive care. Note: All medical staff will be privileged and credentialed according to the rules and regulations of the client.
