## Description **Knowledge, Skills, and Abilities** - Knowledge of professional practice standards, ethics, and code of conduct. - Knowledge of fundamentals, challenges and needs of vulnerable and underserved populations, specifically those at greater risk of contracting HIV. - Demonstrate cultural humility and sensitivity in working with individuals from a variety of racial, ethnic, cultural, socioeconomic, and religious/spiritual backgrounds. This specifically includes members of the LGBTQ+ and Gender Variant/Gender Non-Conforming communities, as well as persons experiencing homelessness, and otherwise vulnerable and at-risk individuals. - Demonstrate knowledge and application of HIV Primary Care Practice and principles and processes of Trauma-Informed Care. - Must be comfortable with, and demonstrate confidence in, engaging patients in sensitive conversations concerning sex and sexuality, and sexual behavior and practices within the context of prevention and health promotion. - Knowledge of medical and mental health and substance abuse diagnoses and treatment procedures services (based on experience and within parameters of license, if applicable). - Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services and ability to effectively coordinate those services. - Requires independent judgment and skill in utilizing supportive, problem-solving or crisis intervention techniques when working with patients and families. - Ability to work and communicate effectively with patients, providers, and coworkers, operating in a team-based, patient-centered environment. - Ability to perform job responsibilities in an effective and appropriate manner according to CFHC core values and code of conduct. - Ability to analyze, prioritize, and organize activities to meet established deadlines and to provide timely customer service. - Ability to follow oral and written instructions clearly and precisely. - Ability to maintain objectivity and sensitivity to persons of all social, cultural, economic and educational backgrounds - Must be able to sit or stand for prolonged periods of time, bend, lift, and move intermittently during the work day. - This position may require extensive travel in support of funded programs, as well as attendance at relevant program-related conferences at both the regional and national levels. - Additional specific tasks of this position include, but is not limited to, the following: - Support PrEP access through care coordination. - Improve PrEP access and adherence, evaluation for co-occurring conditions, and necessary follow up. - Support PrEP adherence through care integration and coordination support that address co-existent behavioral health conditions and social determinants of health. - Support PrEP adherence through the provision of patient education and counseling, and collaborating with community-based organizations working with at-risk populations. - Leverage partnerships to support data-driven quality improvement of PrEP and other prevention services through such strategies such as strengthening information exchange with health departments regarding referrals and re-engaging patients in care, and using pharmacy data on PrEP prescriptions filled to promote adherence. - Organize and participate in community health fair events to attract and enroll community members and raise awareness of HIV, PrEP, post-exposure prophylaxis (PEP), and how to reduce HIV infection risk **Qualifications/Education Requirements** - To qualify for this position, a bachelor’s degree in social work, community health, or related social or behavioral science. - Minimum Experience: 2 – 3 years of experience in a health care setting preferred.
