Our goal is to foster inclusive environments that celebrate diversity in the workforce and social structures, setting a model for other communities across the state and nation. ## Position Summary The Placement Specialist implements all aspects of the Individual Program Plan (IPP), including intake, assessment, goal development, job training, job placement, job coaching, and required documentation. This role also develops and maintains employer and commercial business accounts for Entrusts products, services, and staffing operations. The Placement Specialist ensures compliance with agency philosophy, accreditation standards, and all applicable local, state, and federal laws. * * * ## Essential Duties and Responsibilities ### Individual Program Planning & Service Delivery - Conduct intake, assessments, and work evaluations for participants; develop IPPs and training plans aligned with individual goals. - Facilitate ISP/IPP meetings and collaborate with families, funding agencies, and service providers. - Perform periodic reassessments; update IPPs and maintain accurate, timely documentation. - Intake and exit participants according to established procedures and coordinate with leadership regarding service openings. ### Training, Job Coaching & Progress Monitoring - Implement training plans and monitor participant progress toward employment goals. - Maintain accurate participant files and enter service hours in Setworks within CMIS reporting timelines. - Recommend and implement revisions to training methods to improve job performance and work behaviors. - Educate employers and coworkers on effective support, supervision, and communication strategies. - Schedule and provide ongoing support services to maximize approved service hours. - Assist participants in overcoming barriers to employment, including attendance, behavior, hygiene, or personal challenges. - Provide advocacy services to protect participant rights and promote awareness of the capabilities of individuals with disabilities. ### Individual Supported Employment (ISE) - Orient participants to job sites, policies, coworkers, benefits, and workplace expectations. - Teach self‑management and independence skills. - Assist employers with job restructuring or environmental modifications. - Develop and implement fading plans to transition support to natural workplace supports. ### Group Supported Employment (GSE) - Assess workstations and recommend or assist with accommodations and assistive devices. - Provide or coordinate transportation and mobility training as needed. ### Sales & Business Development - Develop and maintain employer relationships; identify and pursue new business and employment opportunities. - Conduct outreach, presentations, and follow‑up with prospective employers. - Maintain employer databases and submit required weekly production, sales, and marketing reports. - Submit new contract proposals and adhere to customer quality guidelines. - Participate in sales team goal setting, policy development, and team activities. ### General Responsibilities - Communicate regularly with families, care providers, and funding agencies. - Act as an advocate for Entrust participants to ensure rights and privileges are respected. - Identify and refer participants to external services as appropriate. - Coordinate service coverage during absences. - Perform other duties as assigned. ## Qualificati…
