The outpatient therapy team plays a critical role in supporting patient care by providing therapy evaluations and treatments to a wide variety of patients in the Adirondack Region. **How You Will Fulfill Your Potential (Responsibilities)** **Responsibilities** - **Assess Patients:** Evaluate patients of all ages, get to know their specific needs, and partner with them and their families to set realistic goals. - **Provide Hands-On Therapy:** Run treatments using targeted exercises and specialized equipment, adjusting your approach on the fly based on how the patient is progressing. - **Keep the Team in the Loop:** Write up clear, timely progress notes and discharge summaries, and collaborate directly with doctors and nurses on care plans. - **Educate and Guide:** Teach patients and families about their recovery process so they feel confident and safe managing their health. - **Mentor Others:** Help supervise PT students, aides, and volunteers, and share your knowledge with the rest of the therapy team. **What You’ll Bring (Qualifications)** - **Education:** A Bachelor's or Master's degree in Physical Therapy from an accredited program - **Licensure:** An active New York State Physical Therapist license (or eligibility to get one) and a passed state exam - **Certification and Licensure:** Current Basic Life Support (BLS) (or obtained at time of hire) and a valid NYS Driver's License. - **Skills:** - **Clinical Knowledge**: Solid understanding of physical therapy treatments and medical terminology. - **Patient Assessment**: Ability to evaluate clinical data and create clear recovery goals. - **Problem-Solving**: Confidence to make independent decisions. - **Communication**: Warm, clear style for explaining evaluation results to families. - **Teaching Ability**: Skill to instruct patients of all ages on safe movement and exercises. - **Tech Comfort**: Experience using computers and standard office or clinical documentation software. ### About the Company **Mission** Our mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. **Communities We Serve** Located in the foothills of the beautiful Adirondack mountains, Glens Falls is conveniently located a short drive away from the capital region and Lake George.
