In this role, you will evaluate, develop, and implement individualized treatment plans that help patients restore function, improve mobility, reduce pain, and maximize independence. As a valued member of our multidisciplinary rehabilitation team, you will collaborate with physicians, nursing staff, occupational and speech therapists, case management, and rehabilitation support staff to deliver exceptional, patient-centered care. Whether working with inpatient, outpatient, or long-term care patients, you'll have the opportunity to make a meaningful difference in the lives of those you serve. ## Essential Responsibilities - Evaluate patients and develop individualized treatment plans. - Provide skilled physical therapy treatments using evidence-based techniques. - Monitor patient progress and modify treatment plans as appropriate. - Educate patients and caregivers on home exercise programs, mobility, safety, and use of assistive devices. - Collaborate with physicians and interdisciplinary healthcare teams to optimize patient outcomes. - Complete accurate and timely clinical documentation. - Participate in discharge planning and continuity of care. - Mentor Physical Therapist Assistants, students, and rehabilitation support staff as appropriate. - Participate in quality improvement initiatives and continuing education. - Maintain compliance with HIPAA, accreditation standards, and HMC policies. ## Qualifications - Graduate of an accredited Physical Therapy program (DPT preferred). - Current Illinois Physical Therapist license or eligibility for licensure. - Current CPR/BLS certification or ability to obtain upon hire. - One to two years of clinical experience preferred; new graduates are encouraged to apply. - Strong clinical assessment, communication, and critical thinking skills. - Ability to work effectively within a collaborative healthcare team. - Commitment to delivering compassionate, patient-centered care. ## Why Join Hopedale Medical Complex?
