Our team is growing and looking for a dedicated: **Physical Therapist** **(PT)** ***Shifts Available:* Full-Time, Part-Time & PRN** ***Service Areas with Openings: Northeast, Northwest*** **Responsibilities:** - Facilitate the rehabilitation of patients with emotional, physical or mental disabilities. - Implement physical therapy care plans. - Coordinate care with the home healthcare team. - Educate patients and family members about health conditions. - Prepare timely, complete, and accurate clinical notes. - Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members. **Qualifications:** - **Physical Therapist in the state of Texas.** - Home health experience highly preferred. - Knowledge of home health regulations and general healthcare compliance. - Experience with HCHB EMR.
