If you're passionate about making a meaningful impact while enjoying autonomy, flexibility, and a supportive team environment, we'd love to hear from you. ## What You'll Do As a Home Health Physical Therapist, you will: - Perform comprehensive initial evaluations to assess patient needs and develop individualized treatment plans. - Design and implement evidence-based therapy programs to improve mobility, strength, balance, function, and overall quality of life. - Monitor patient progress, adjust treatment plans, and ensure optimal outcomes. - Educate patients and family members on exercises, safety techniques, and home programs. - Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care. - Maintain accurate, timely clinical documentation in accordance with company policies and regulatory requirements. - Follow infection control and patient safety standards during every visit. - Deliver compassionate, patient-centered care while promoting our mission and values. ## Qualifications - Graduate of an accredited Physical Therapy program. - Current, unrestricted Physical Therapist license in the state of practice. - CPR Certification. - Valid driver's license and reliable transportation with current auto insurance. - One (1) year of Home Health experience preferred. - Homecare Homebase experience preferred. ## Physical Requirements - Ability to stand, walk, sit, bend, kneel, squat, crawl, and climb throughout the workday. - Ability to safely lift, carry, push, and pull up to 100 pounds with or without reasonable accommodation. ## Benefits & Perks We value our team members and offer a comprehensive benefits package, including: - Medical, Dental & Vision Insurance - 401(k) after just 3 months of service - 100% immediate vesting - Company matches **100% of the first 3%** you contribute each pay period - Health Savings Account (HSA) - Life Insurance - Short-Term & Long-Term Disability Insurance - Paid Time Off (PTO) - Paid Holidays - Mileage Reimbursement Stipend
