**Physical Therapist (PT) - Home Health** - **Schedule:** Full-time Monday–Friday + occasional weekends (as needed) - **Location:** Field Based - **Compensation (Productivity Points):** ***28 PP*** ### Who We Are: Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas.
