**Physical Therapist All Heart Home Care | Nashville Area | FT** **All Heart Home Care** is seeking a compassionate and skilled Physical Therapist (FT) to join our home health team in the Murfreesboro area. This role offers flexible, as-needed scheduling while providing personalized, one-on-one care to patients in the comfort of their homes. **Key Responsibilities:** - Perform in-home evaluations to assess patient needs and develop individualized treatment plans - Provide skilled physical therapy services to improve mobility, strength, balance, and overall function - Monitor patient progress and adjust treatment plans as appropriate - Educate patients and caregivers on home exercise programs, safety, and injury prevention - Complete accurate and timely clinical documentation in compliance with company policies and state regulations - Coordinate care with physicians, nurses, case managers, and other members of the healthcare team - Travel to patient homes within assigned service areas **Qualifications:** - Doctor of Physical Therapy (DPT) or equivalent from an accredited program - Active Physical Therapist license in the state of Tennessee - Current CPR certification - Valid driver's license, reliable transportation, and current auto insurance - Strong clinical assessment and communication skills - Ability to work independently and manage time effectively - Previous home health experience preferred but not required **Work Schedule:** - Full Time **About All Heart Home Care:** At All Heart Home Care, we are committed to providing high-quality, compassionate care to patients in their homes.
