Phlebotomist- Safety Sensitive **JOB DUTIES:** **** 1. Performs venipuncture, finger sticks, and heel sticks on inpatients, outpatients, and Pre-Admission Testing patients in such a manner as to obtain optimum blood specimens at a minimum of patient inconvenience. 1. Ensures that specimens are collected per physician’s order and returned to the laboratory appropriately to maintain optimal turnaround of all test results per department policy. 1. Identifies all patients by use of hospital identification, nursing identification, or statement from patient of full name or identification by relative. This is done prior to collection of all specimens. 1. In the presence of the patient label all specimens with patient’s full name, date, time, and initials of the phlebotomist. 1. Make notation on comment line of order in computer of any information necessary for the person performing the test. Example: when blood must be collected by finger stick or heel stick, or is drawn above an IV. 1. Processes specimens for analysis by centrifugation, uses judgement in assessing specimen suitability. 1. Performs Quality Control as required and immediately reports any outliers to a registered Medical Technologist or supervisor. 1. Patient Data Entry 1. Does admission on all Continuous Care or Standing Order patients with accuracy and efficiency on each visit. 1. Enters physician’s orders into Laboratory Information System, including tests being ordered and any comments such as fax or call comments. 1. Checks that all information on the patient data sheet is correct and has been entered as stated on the physician’s written order. 1. If medical necessity tests are ordered, checks diagnosis for insurance coverage of lab tests ordered. Requests that patient sign Advanced Beneficiary Notice (ABN) when applicable. Gives a copy of the ABN to patient, sends a copy each to Medical Records and Patient accounts, and keeps a copy for Outpatient Lab records. Documents in computer under “ABN Log Menu” that an ABN has been signed. 1. Prints “Collected but not logged in” report and investigates outstanding orders as needed. 1. Inventories and orders Drug Screen Collection Kits/Forms for Outpatient Lab and Emergency Department. 1. Monitors standing orders for Continuous Care patients and requests new orders from physician when current order is six months old. 1. Assists as assigned in delivering patient reports to the proper destination. 1. Maintains clean and well stocked work area. 1. Maintains, stocks, and inventories supplies and reports needs to phlebotomy supervisor. 1. Maintains a clean work area. Cleans phlebotomy chair and blood collection station, and Outpatient Lab countertops according to OSHA guidelines and lab infection control and safety policies. 1. Follows hospital and departmental policies and procedures. 1. Meets attendance expectations as specified in the White River Medical Center’s Personnel Policies and Procedures Manual. 1. Attends mandatory hospital inservice concerning Infection Control and Safety. 1. Upon employment, attends inservice training and or reviews film on methods of obtaining blood from pediatric, adult, and geriatric patients. Reviews new procedures and products pertinent to drawing of various patients as needed. 1. Attends or reads minutes of each departmental meeting. 1. Attends all departmental inservices. 1. Follows OSHA guidelines for safety. 1. Accepts and performs other assigned duties with a positive attitude, as requested and documented by the supervisor. 1. Has demonstrated or provided proof of meeting all mandatory competencies. 1. Has met all employee health requirements for the evaluation year and conforms to established dress, protective equipment, and patient isolation policies. 1. Interacts in an effective, willing, professional manner with patients, guests, and other health care team members. 1.
