As part of the terms of employment, employees in this role are required to join the union within 31 days of hire and remain a member (e.g. dues paying, fee paying, religious exception contributor) for the duration of the collective bargaining agreement. **This position was originally posted to ratified SEIU members from 06/11/2026 – 06/19/2026. The position is now available to be filled by internal candidates that are not members of the ratified Bargaining Unit or external candidates to Sharp.** **What You Will Do** Under the supervision of the Pharmacy Department, the Medication Reconciliation Technician is primarily responsible to obtain and document an accurate patient medication history profile that supports the mission, values and philosophy of Pharmacy Services and Sharp HealthCare. **Required Qualifications** - 2 Years experience in a pharmacy or similar environment. - California Pharmacy Technician License (TCH) - CA State Board of Pharmacy -REQUIRED **Preferred Qualifications** - Combination of both inpatient and outpatient experience preferred, particularly outpatient experience. - Certified Pharmacy Technician (CPhT) - Pharmacy Technician Certification Board -PREFERRED **Essential Functions** - Customer service and teamwork Demonstrates a commitment to serving other employees and patients through teamwork, collaboration, developing others and an uncompromising adherence to SHC's values. Decisions reflect the Sharp HealthCare Mission, Values and Philosophy. - Department services Demonstrates content expertise regarding medication reconciliation. Participates in task forces, committees at discretion of supervisor. Assist in developing materials to support education initiatives. Assist in ongoing IS improvement projects regarding Medication Reconciliation applications. Ensures that patient's medication reconciliation gets completed in a timely manner. Assist in ongoing development of process improvement to improve throughput. - Medication reconciliation and patient education The technician will comply with all related state and federally mandated requirements in performing the job assignment. All interviews of patients will be conducted in accordance with State and Federal Regulations. All medical consultations will be referred to Medical Staff, Nursing Staff or Pharmacist staff to complete. - Medication safety Demonstrates awareness of medication safety communications delivered by pharmacy management and safe medication practice pharmacist. Qualitative work demonstrates thoughtful and thorough identification, evaluation and analysis of medication safety events. Quantitative work reflects the desire to contribute to the ADE monitoring aspect of Safe Medication Practice. - Productivity Completes appropriate number of medication reconciliation profiles during shift.
