A Pharmacy Technician in Care Management plays a crucial role in supporting healthcare teams to ensure patients receive safe and effective medication therapy. Address patient concerns or refer them to the appropriate healthcare provider. **Medication Adherence Monitoring** Track patient adherence using electronic health records (EHR) and pharmacy tools. Follow up with patients struggling with adherence and report concerns to the care team. **Care Coordination** Communicate with healthcare providers to support care transitions, such as hospital discharges. Assist with medication refills, prior authorizations, and delivery coordination. **Data Entry & Documentation** Accurately update patient records in EHR systems. Document interactions with patients and healthcare teams. **Support for Pharmacists & Healthcare Teams** Assist in medication reviews, identify potential drug interactions, and support medication safety initiatives. Provide reports on medication usage to aid in care planning. **Compliance & Quality Assurance** Ensure adherence to HIPAA, state pharmacy laws, and regulatory requirements. Participate in quality improvement initiatives to enhance patient care. **Required Skills & Qualifications** **Certification: Certified Pharmacy Technician (CPhT) through PTCB or NHA.** Experience: Prior experience in a pharmacy setting, preferably in care management. Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and chronic disease management. Technical Skills: Familiarity with EHR systems, pharmacy software, and telehealth tools. Communication: Excellent verbal and written skills to effectively collaborate with patients and healthcare teams. Attention to Detail: Accuracy in documentation and medication processing. Problem-Solving: Ability to identify and resolve medication-related concerns with the care team. A background check is required for this role.
