You will: β Work closely with patients to deliver a full continuum of medication adherence support using our tools and applications β Encourage patients of specialty clinics to utilize pharmacy services for superior care β Seamlessly integrate with the clinic team (doctors, nurses, staff) as youβll be located in one of the medical center clinics π Job Duties π Provide βlegendaryβ care to hospital patients with custom medication management services π©ββοΈ Work alongside hospital clinicians to serve patients face-to-face and over the phone π Educate prospective pharmacy patients on how hospital services improve care π Support outpatient retail and specialty prescription needs π Grow the specialty pharmacyβs patient population π Ensure patients stay on track with medication regimens (outbound check-ups, securing refills, etc.) π Communicate effectively with the centralized Patient Service Center regarding reports and lists π Help develop new processes for smooth operations and patient care β Identify medications requiring special storage (hazardous/refrigerated) π§° Identify necessary supplies (sharps containers, syringes, alcohol swabs, Band-Aids) π₯ Answer, resolve, and triage inbound inquiries β Perform refill reminder calls under pharmacist supervision π³ Resolve insurance issues, assist with prior authorizations (PAs), and financial assistance π₯ Ensure accurate documentation in all systems π Manage and update data using database applications π‘ Apply observations to improve productivity, quality, and service standards π Identify trends, resolve problems, recommend improvements π Actively participate in process improvement initiatives π€ Act as a value-added business partner to stakeholders π Protect organizationβs value by maintaining confidentiality π Update knowledge through education, publications, and networking β
