**Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN)** **Conroe, TX | Bilingual Preferred (Not Required) | Full Time** **Pay Range:** - **LVN:** $30–$35/hour - **RN:** $35–$42/hour * * * **Make a Difference Beyond One Shift** **Angels of Care Pediatric Home Health** is hiring **Registered Nurses (RN)** and **Licensed Vocational Nurses (LPN) for our Pediatric Transitional Nurse Team** to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with **high-acuity patients and** **skilled in trach, vent, G-tube, and emergency management**. Nurses in this position play a critical role supporting families, training caregivers, and ensuring safe continuity of care in the home environment. The company was founded by a nurse with a huge heart, and today we pride ourselves on our values: **H**eart, **A**dvocacy, **L**ove, and **O**utreach! **#HALOS** * * * **What You’ll Do** - Provide skilled pediatric nursing 1:1 care in patients’ homes - Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies - Support hospital-to-home transitions for high-acuity patients - Educate and support parents, families, and caregivers - Assist with discharge support and care coordination - Train and support permanent field nurses during transitions - Provide first-shift and interim coverage as needed - Collaborate with Clinical Care Managers and interdisciplinary teams - Respond appropriately in emergency situations and changes in patient condition * * * **Qualifications** **Required:** - Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license - At least 1 year post license certification - Current CPR certification - Ability to lift and assist pediatric patients as needed - At least 3 years of experience with Trach care, Ventilator management, and TPN, post-graduation - Emergency response management - Pediatric home health experience - High-acuity pediatric patients (preferred) * * * **Schedule & Travel** - 12 hour shifts (Day or Night) - Flexible scheduling based on patient needs and Nurse availability - 5 working days per week, minimum 1 weekend day - On call and Staffing coverage responsibilities as needed - Local travel within approximately 30–40 miles * * * **Why Angels of Care:** We have a full benefits package, bulleted out below.
