Patient Care Liaison - Medical

The **Patient Care Liaison** is a cross-functional healthcare support professional responsible for enhancing patient access, supporting medical workflows, assisting with continuity of care activities, and maintaining accurate documentation within a federally aligned primary care clinic. This position serves as a vital connection between patients, providers, clinical staff, and external healthcare organizations to ensure a seamless, patient-centered healthcare experience. The Patient Care Liaison combines administrative, patient service, and clinical support functions while helping patients navigate healthcare services and supporting providers in delivering high-quality, coordinated care. **Duties to include:** - Welcome and assist patients in a professional, compassionate, and confidential manner. - Schedule, confirm, and coordinate patient appointments. - Verify demographic, insurance, and registration information. - Coordinate referrals to specialty providers and community resources. - Track referral status and obtain consultation reports. - Prepare patient charts and ensure documentation is complete prior to appointments. - Support providers with administrative and clinical workflow tasks. - Ensure patient information is documented accurately and timely. - Maintain confidentiality and comply with HIPAA regulations. - Serve as a liaison between patients, providers, behavioral health staff, nursing, care management, and external healthcare organizations. - Participate in team meetings and collaborate to improve patient outcomes. - Support clinic operations by assisting with special projects and other duties as assigned. **Minimum Requirements:** High school diploma or equivalent required, Medical Assistant certification preferred. Minimum of 2 years’ experience in: Medical office operations Front desk registration, Clinical support or Medical Assisting. Experience in using electronic medical record systems required.
