Patient Care Coordinator (RN) – Outpatient Provider Based Clinics

## Description **Position Summary** The Patient Care Coordinator (RN) manages and coordinates patient care across our network of hospital employed provider outpatient clinics. This role ensures seamless continuity of care for patients traveling to access specialty services. You will serve as the primary clinical liaison between patients, primary care providers, regional clinic staff, and the specialty physician. This position combines clinical expertise while rooming patients, case management, and care coordination to optimize patient outcomes and streamline clinic operations.**** Key Responsibilities Clinical Care Coordination: - Room all patients and assist with procedures. - Act as the primary point of contact for the patients and providers of the hospital outpatient clinics. - Review patient charts, provider notes, and diagnostic results prior to clinic dates, ensuring testing has been ordered and completed for the office visit .. - Coordinate scheduling via the work queue as well as lab work, imaging, and provider visits. Set up surgical procedures as requested. - Facilitate post-visit follow-up, ensuring patients understand their care plans, medications, and next steps. - Order supplies and keep all areas stocked and ready at all times. Quality, Safety, & Compliance: - Ensure all outlying clinic workflows comply with organizational policies, HIPAA regulations, and healthcare standards. - Track and report key patient metrics, including clinic utilization, travel barriers, and care adherence. - Identify gaps in regional care delivery and propose process improvements to leadership. **Schedule:** Full-Time 32 hours weekly, 8 hours shifts, M-F day shift ## Requirements Qualifications & Skills - Education: Associate Degree in Nursing (ADN) required. - Licensure: Active, unrestricted Registered Nurse (RN) license in the state of practice. - Certification: BLS (Basic Life Support) certification required. ACLS required, can obtain once employed. Experience - 2–3 years of clinical nursing experience preferred. Skills & Competencies - Clinical Judgment: Strong assessment and triage skills with the ability to work independently away from the main campus. - Communication: Exceptional verbal and written communication skills for managing distant provider relationships. - Organization: Advanced multitasking abilities to coordinate schedules across multiple geographic locations. - Technology: Proficiency with Electronic Health Records (EHR) systems and telehealth platforms.
