(FT/PT) Our Mission: We Celebrate and Enhance All Lives with Kindness & Integrity Highlights of the Memory Care Coordinator Position: The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the MCD/HWD in non-clinical health services. Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community.
