The position reports to the PACE Program Director. The primary responsibility of the PACE Community Outreach Specialist is the ongoing recruitment and enrollment of PACE participants into the program and the growth and maintenance of an optimal enrollee census. In addition, this position is responsible for all community awareness efforts of the program, including the marketing of services to providers, referral sources, and the community at large. Primary Accountability The position reports to the PACE Program Director. The primary responsibility of the PACE Community Outreach Specialist is the ongoing recruitment and enrollment of PACE participants into the program and the growth and maintenance of an optimal enrollee census. In addition, this position is responsible for all community awareness efforts of the program, including the marketing of services to providers, referral sources, and the community at large. Description of Primary Responsibilities: 1. Recommends, implements, and evaluates the overall marketing strategy for recruiting and retaining PACE participants and building and maintaining an optimal enrollee census. 2. Plans and coordinates advertising and promotional campaigns and strategies for the PACE Program. 3. Coordinates the production of printed publications and audio-visual productions, including developing design, layout, and text for informational materials such as newsletters, bulletins, pamphlets, directories, and audio-visual productions. 4. Initiates, develops, and maintains public contacts for disseminating program information. Drafts news releases that are submitted for higher-level approval through FHCN’s Marketing department 5. Identifies potential public relations problems and advises program leadership and agency officials of appropriate preventative actions. 6. Coordinates ceremonies, press conferences, and other events or special projects. 7. Maintains administration and relevant reporting, tracking, and planning systems. 8. Plans and coordinates all marketing resources within an approved budget. 9. Responsible for supporting the health care team through community outreach, enrollment assistance, and education activities, as assigned. 1. Provides information on health insurance, safety programs, and community resources. 2. Conducts intake interviews to determine eligibility for PACE. 3. Attends relevant community meetings as assigned. 4. Develops recommendations for improvements to the community healthcare system. 10. Organizes and participates in network-hosted events, community events, and presentations. 1. Provides training on health-related topics as assigned. 2. Provides health screenings at events. 11. Responsible, as assigned, to support the implementation of PACE programs and grant-funded initiatives. 1. Supports the implementation of program curriculum and/or objectives in community settings. 2. Supports supervisor in the collection of data and reporting. 12. Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law. 13. Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need. 14. Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law. 15.
