Location: San Jose Northeast and Northwest Scheduled: Availability: 3 weekdays and 1 weekend per month. Responsibilities Provide occupational therapy treatments as ordered by the physician Assist in developing and updating the Plan of Care (POC) Evaluate patient functional levels and progress Observe, record, and report patient condition changes Teach and guide family members and Lorian staff on therapy programs Supervise other healthcare personnel when appropriate Assess the home environment and recommend modifications Participate in in‑service education programs Complete and submit accurate, timely clinical documentation per Lorian policies Qualifications • Registered by the Occupational Therapist Association and licensed in the state of practice if the state has registered practices. Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. • Possesses and maintains good physical stamina and mental health. Presents a pre-employment.physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.• Has the ability to read and follow written instructions and document care given.• Is self-directing and has the ability to work with little direct supervision.• Possesses and maintains current CPR certification.• Is flexible and cooperative in fulfilling the role obligations.• Demonstrates tact, patience, and good personal hygiene.• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.• Must be able to document all client-related activity practices.
