Occupational Therapist – Reablement and Home First

Liquid Personnel is seeking an experienced Occupational Therapist to support one of its clients in meeting increased demand following the successful rollout of their Home First contract. This is a high impact reablement role, focused on enabling individuals to maintain independence and reducing the need for long-term care support. What will your responsibilities be? Complete strengths-based assessments in line with Care Act principles Develop reablement-focused care plans to maximise independence Provide expert advice on equipment and adaptations Manage a diverse community caseload, including home visits and hospital settings Work closely with multi-disciplinary teams to support safe discharges and prevent admissions Maintain accurate records and contribute to service outcomes Benefits: Excellent rate of £38 p/h Flexible hybrid working model Autonomy to manage your own diary Work within a forward-thinking Home First service Strong chance of extension Qualifications & Experience: Qualified Occupational Therapist with HCPC registration Minimum 2 years post-qualified experience Experience within: Reablement / Community OT Adults Social Care Full UK driving licence (essential) Working Pattern: 37 hours per week ~50% of admin completed in the office (County Hall) Remaining time split between: Home working Community visits Example structure: 3 days visits 1 day office-based 1 day working from home 4-day working week considered (no less) Why Liquid Personnel?
