- **Role:** Occupational Therapist (OT) - **Schedule:** Full-time Monday–Friday + occasional weekends (as needed) - **Location:** Field-based - **Comp:** 28PP ### Who We Are Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our ultimate mission is to **dramatically improve patient access to home health care.** The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs. We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our **charting product** is designed to reduce documentation burden and keep visits moving, and our **scheduling product** (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time. If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction. ### What you will do - Complete in-home OT evaluations and deliver skilled OT visits. - Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence. - Recommend and train on adaptive equipment and home modifications as needed. - Coordinate with nursing and the broader care team when condition or needs change. - Document promptly and thoroughly in **Homecare Homebase (HCHB)** — defensible, efficient, and consistent. ### Why Adaptive - **Operations that back you up:** You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics. - **Clear productivity model with upside:** You are paid per productivity point with clear expectations (market-specific). - **Schedule flexibility:** We build schedules to make field work sustainable, and we support smart routing and coordination so your day is predictable whenever possible. - **Cutting-edge tech built for clinicians:** Our charting tools are designed to reduce documentation burden, and our scheduling product (launching soon) will streamline visit coordination and reduce last-minute changes. - **Mission-driven work:** We are here to expand access to high-quality home health care, and we want therapists who take pride in raising the standard. ### Requirements **Must-haves** - Active **Texas OT license** (in good standing) - Strong clinical judgment and comfort working independently in the field - Comfort escalating changes in condition to RN/MD and documenting rationale clearly - Strong communication and documentation - Reliable follow-through on orders, frequencies, and patient goals - Current driver’s license and comfort with daily travel across assigned territory - Able to carry/transport basic therapy equipment as needed - Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education) **Nice-to-haves** - Prior **home health** experience - **Homecare Homebase (HCHB)** experience (or similar home health EMR) - Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training) - Strong organization and time management
