# Occupational Therapist (OT) – PRN ## Ascend Home Health | Southern Idaho Ascend Home Health is seeking a compassionate and motivated **Occupational Therapist (OT)** to join our team on a **PRN basis** serving patients throughout Southern Idaho. This role offers flexible scheduling and the opportunity to make a meaningful impact by helping patients regain independence, improve daily functioning, and safely remain in the comfort of their homes. ## Responsibilities: - Evaluate and treat patients in their homes according to physician orders and plan of care - Develop individualized therapy treatment plans focused on improving daily living skills and independence - Educate patients, families, and caregivers on therapy techniques, safety, and adaptive equipment - Collaborate with nurses, therapists, physicians, and the interdisciplinary care team - Complete timely and accurate documentation ## Qualifications: - Current Occupational Therapist license required - Home health experience preferred, but not required - Strong communication and organizational skills - Compassionate, dependable, and patient-focused - Valid driver's license, reliable transportation, and current auto insurance required ## Position Details: - PRN / Flexible scheduling - Coverage area throughout Southern Idaho - Competitive pay per visit DOE (depending on experience) At Ascend, we are committed to providing compassionate, personalized care while supporting our employees with a positive and collaborative work environment.
