Nurse Practitioner – In-Home & Telehealth Care

Position Overview ACE Physicians is expanding into Tyler Texas, and we are looking for compassionate and motivated Nurse Practitioners to help bring high-quality, patient-centered care directly to the communities we serve. This hybrid role provides care through **in-home visits and telehealth**, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable. Key Responsibilities - Perform comprehensive health assessments and physical examinations during **in-home visits and telehealth appointments**. - Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care. - Develop and implement individualized treatment plans tailored to each patient’s needs. - Educate patients and families on health management, wellness, and disease prevention. - Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care. - Perform minor clinical procedures such as wound care as appropriate during home visits. - Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms). - Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities. Qualifications - Active, unrestricted Nurse Practitioner license (required). - Experience in geriatrics, internal medicine, or primary care preferred. - Previous home health or home-based care experience strongly preferred. - Experience caring for patients with disabilities is highly desirable. - Comfortable delivering care in both **in-home and telehealth settings**. - Excellent communication skills and a collaborative, team-oriented mindset.
