**Nurse Practitioner (NP) – PACE** **$15,000 Sign-On Bonus and a 4-day work week!** ***HopeWest PACE is seeking Primary care or Geriatric experienced Nurse Practitioner to join a robust interdisciplinary team, to collaborate in developing and implementing comprehensive care plans while enhancing the lives of medically complex elderly individuals.*** ***Our Mission Statement****: “To profoundly enhance the everyday lives of frail, elderly, individuals by supporting their independence, increasing their social support and providing comprehensive, well-coordinated whole-person care”* **What is PACE?** PACE (**P**rogram of **A**ll-Inclusive **C**are for the **E**lderly) was created to answer the many problems around caring for frail and medically complex seniors. The PACE model is intended to keep individuals living safely and comfortably in their homes whenever possible. HopeWest PACE participants receive wrap around services that include but not limited to in home care services, transportation to a HopeWest PACE center for primary medical care, social and recreational activities. **The Unique Responsibilities of the Nurse Practitioner at** **HopeWest** **PACE:** - Provides a wide range of Primary Care services including diagnosis and treatment of chronic and acute illness, preventative care, palliative, and urgent medical needs all in collaboration with the PACE Medical Director. - Conducts health assessments and evaluates patient needs in the clinic, the PACE Day Center and the patient’s home. - Actively collaborates with the entire PACE interdisciplinary team for the highest quality care of each participant. - Make appropriate and timely referrals to our contracted providers out in the community. **What You Bring:** - Two (2) years of Nurse Practitioner clinical experience preferred. - Experience working with older adults, individuals with multiple chronic conditions, or medically complex individuals. - Experience in primary care, hospice, palliative care, home health, long term care or PACE preferred. - Current and unrestricted APN and RXN license in Colorado required. - National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or other appropriate population focus preferred. - Controlled substance license with current DEA registration or eligibility to obtain. - Current CPR/BLS certification maintained throughout employment. **What We Offer:** - Pay structure of $112,798.40 - $124,800 annually based on experience. - UMR Health, Delta Dental, and VSP Vision insurance effective the month following start date - Immediately begin accruing PTO and medical leave - 15% discount at Spoons Bistro & Bakery and all Heirlooms locations. - Up to 3% match on 403B retirement - Employer paid life insurance and Employee Assistance Program - Mileage reimbursement for work-related driving.
