The **Licensed Practical Nurse** will perform the following tasks: - Provide direct nursing care to clients under the supervision and direction of clinical leadership. - Maintain patient confidentiality in accordance with Phoenix House policies and procedures. - Oversee unlicensed assistive personnel (e.g., CNAs, Medical assistants, and Medical Technicians). - Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies. - Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership. - Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership. - Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies. - Other duties as assigned. **REQUIRED EDUCATION** - High School Diploma or GED - Graduate of an accredited nursing program **LICENSES/CERTIFICATIONS** - Current NYS Licensed Practical Nurse - First Aid Training - CPR Training - Narcan Training - Infection Control Certification - Child Abuse Certification **REQUIRED EXPERIENCE** - At least three years of related experience - Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs. **COMPETENCIES** - Administration of subcutaneous and intramuscular injections - EKG proficienc…
