We believe in the dignity of every patient and every team member, and we empower our providers to practice at the top of their license. **The Role:** As a **Nurse Practitioner/Physician Assistant**, you will play a crucial role in delivering high-quality, whole-person care to patients experiencing chronic pain, depression, or substance use disorders. Working directly out of our main office, you will collaborate closely with a multi-disciplinary team in a supportive, fast-paced environment. This role offers the perfect balance of clinical independence and immediate, on-site peer collaboration. **Key Responsibilities:** - Perform thorough patient assessments, formulate accurate diagnoses, and develop individualized treatment plans. - Manage same-day patient visits and ensure all documentation is completed accurately and timely (within 72 hours). - Provide in-house treatments such as trigger point injections and pharmacy refills. - Facilitate appropriate referrals for imaging services, Physical Therapy, Chiropractic care, Orthopedic consultations, and laboratory services. - Work collaboratively with the team to ensure seamless care delivery, efficient clinic flow, and robust patient support. **What You Bring:** - Current and unrestricted **Nurse Practitioner/Physician Assistant** license in Maryland. - Active CDS and DEA licenses. - Experience in pain management, mental health, behavioral health, or addiction medicine is preferred. - Demonstrated strong clinical judgment, excellent time management, and meticulous follow-through. - A patient-first mindset, committed to providing compassionate and comprehensive care. - Comfort and proficiency working independently in a satellite clinic setting, with full remote support. **Why Lifestream?** At Lifestream, we prioritize a mission-driven, non-corporate clinic culture where providers are respected and supported.
