Beacon is a successful and national behavioral health company providing residential-based services and supports to children and adults with Complex Behavioral Health conditions, Severe Mental Illness, Autism, and other compounding medical conditions. All activities related to this position must be delivered in compliance with the company’s policies and procedures, accreditation standards, state and federal licensing requirements, HCBS guidelines, and the contractual requirements of Community Mental Health Agencies and other regulatory bodies. Position Summary: A New Jersey Supports Coordinator is a key partner for individuals enrolled with the Division of Developmental Disabilities (DDD). Their primary role is to guide individuals and families through the DDD system, help them understand available services, and ensure that supports align with the person’s goals, needs, and preferences. They conduct the Person-Centered Planning process, working closely with the individual, family, and care team to develop and maintain the NJ Individualized Service Plan (ISP). This includes assessing needs, identifying service options, coordinating with community providers, and helping manage the individual’s annual DDD budget. Supports Coordinators monitor the quality and effectiveness of services through regular check-ins, plan updates, and required monthly or quarterly contacts. They advocate for the person’s rights, ensure compliance with DDD policies, and assist with transitions—such as entering adult services, changing providers, or navigating Medicaid and other benefits. Ultimately, their role is to empower individuals with intellectual and developmental disabilities to live safely, independently, and meaningfully within their communities. Supervisory Responsibilities: No supervisory responsibilities Primary Responsibilities: • Always be compliant with all company and regulatory policies and procedures. • Monitors for quality service delivery • Addresses family/individual and provider concerns • Attend outside trainings and webinars. • Ensures compliance with all regulating agencies and internal policy and procedures. • Understands internal and external policies and follows employee rules of conduct and performance. • Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities. • Implements Keystone’s mission statement. • Ensure compliance with all Division policies, including provisions related to zero-reject and zero- • discharge, confidentiality, etc. • Ensure that any concerns related to individual health and safety of any program consumers are addressed and communicated to all appropriate parties. • Complete all modifications and updates to the Service Plan as necessary and submit them to the WAC within defined timeframes • Remain informed of current services, supports, provider pool and best practices • Ensures that NJSC’s are adhering to all guidelines in the Provider Agreement and the Supports Program Policy Manual. Team Member Expectations: • Must be compliant with company and regulatory policies and procedures. • Participate and contribute to weekly, bi-week or monthly leadership calls.
