Three 15-hour shifts per week. **Schedule:** Night Shift Hours: 5 PM-8 AM. Work outside of these days/hours may be required. **Pay:** $42.00-60.53 per hour, based on experience. This dynamic role primarily focuses on field-based, on-call responsibilities, delivering timely and compassionate care to patients and families in their homes. When not actively responding to field calls, the RN will provide meal and rest break coverage, as well as light patient care, at our inpatient Hospice House. Additional duties may include assisting with chart audits to support continuous quality improvement across the organization. These secondary duties support continuity of care and uphold clinical excellence. **Essential Functions:** ***Primary Responsibilities-After Hours Field RN Support:*** - Develop an individualized care plan that establishes goals based on nursing diagnoses and incorporates therapeutic, preventive, and rehabilitative nursing actions. Include the patient and the family in the planning process. - Provide nursing procedures and treatments, and/or end-of-life comfort procedures, per the plan of care. Administer medications and treatments as prescribed by the physician. - Complete and update the ongoing assessments and care plan as the patient’s status changes. - Ensure that arrangements for equipment, other necessary items, and services are in place. - Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and the patient’s caregivers’ ability to learn. Adapt teaching styles to meet patients’ and families’ needs. - Coordinate patient care with the patient’s case manager.
