**Mental Health Practitioner I or II - ARC Outreach - Northside Community Support** **People Incorporated Mental Health Services** is seeking an Mental Health Practitioner II - ARC Outreach to join our Northside Community Support Program. The ARC Outreach Team simultaneously embeds **ARMHS and Outreach services for adults**, treating individuals experiencing housing instability and co-occurring mental health illnesses. We utilize a person-centered team approach to help clients meet their goals and better manage overall wellness and mental health. We offer a different approach to homeless outreach, utilizing expertise across multiple programs to address co-occurring conditions of mental health, substance use, and/or other complexities. This role requires candidates to provide street outreach and engage with clients both in the community and at our drop-in center, meeting individuals where they are at. **Schedule:** Full-Time 1.0FTE, Monday - Friday 8:00am - 4:30pm, Flexible schedule **Location:** Northside Community Support **Hiring Range for Mental Health Practitioner I:** $22.50/hour - $23.76/hour, plus $1,000 hiring bonus for external candidates. **Hiring Range for Mental Health Practitioner II:** $24.20/hour - $26.14/hour, plus $1,000 hiring bonus for external candidates. **People Incorporated Mental Health Services** is the largest nonprofit mental health provider in Minnesota. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions. **Main Job Duties:** Client Engagement & Person-Centered Support- - Build and maintain therapeutic, person-centered relationships that promote client recovery and goal attainment. - Maintain regular, active contact with clients to support service utilization and evaluate progress. Service Delivery & Intervention - Facilitate and conduct individual services addressing mental health, behavioral, and wellness needs; provide group interventions as applicable. - Deliver crisis intervention and de-escalation strategies to ensure client and program safety. Service Coordination, Collaboration and Transition Support - Manage and track internal and external referrals from the evaluation process. - Facilitate and coordinate connections to services, including “hand-off” to long-term care coordination teams. Documentation & Record Management - Document all client interactions, services, and interventions in the Electronic Health Record (EHR) or designated systems according to program standards. - Maintain accurate, timely case notes and ensure all activities are properly recorded and compliant. To perform this job successfully an individual must be able to perform each essential duty satisfactorily. Education and Experience **Mental Health Practitioner I - ARC Outreach** - Master’s degree in a behavioral science or related field; OR - Bachelor’s degree in a behavioral science or related field and proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; OR - Has completed 30 semester hours or 45 quarter hours in behavioral science or a related field and: - Has proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; O…
