Arrange for any professional evaluations which may be necessary to further ascertain consumer’s eligibility and service needs. Clarify and explain to consumers the services available through the agency and the County MH/MR Program. - Assesses emergency and “immediate need” situations and makes appropriate referrals. Facilitates voluntary and involuntary commitments and admissions to inpatient psychiatric facilities. - Ensures that consumers undergo an initial financial liability determination and annual liability while on administrative caseload. - Utilizes strengths/needs assessment and environmental matrix to determine level of case management services needed. Completes referrals and authorizations for appropriate levels of case management services. Makes initial referrals to provider agencies. - Arrange for any professional evaluations which may be necessary to further ascertain consumer’s eligibility and service needs for purpose of liabilities. Clarify and explain to consumers the services available through the agency and the County MH/ID Program. Complete referral to County Assistance Office for MA insurance if appropriate. - Follows established protocol and procedures for gathering consumer intake information. - Provide individuals pending TCM service with a bi-weekly phone contact. - For individual’s matrixed on an Administrative level of care: - Arranges, and when necessary, authorizes mental health services. Coordinates and facilitates consumer movement through the Service Delivery System to ensure continuity of care and the provision of services in a timely and organized manner. - Facilitate interagency team meetings and ISPT meetings. - Monitors consumer’s participation and progress in planned services by completing bi- monthly phone contact with individual. - Complete a community face to face level of care assessment every six months for individuals’ matrixed at the administrative level of care. - Complete Matrix, Releases of Infor…
