**Job Opportunity: Blended Case Manager – Mental Health Program** **Location: 1100 S. Additional starting salary incentives available for Master’s degrees and bilingual/ASL skills. - Student Loan Forgiveness: Qualify for student loan forgiveness under the Public Service Loan Forgiveness Program (PSLF). - Supportive Team Environment: Join a mission-driven team committed to exceptional collaboration and support. **What You’ll Do:** **As a Mental Health Case Manager, you will:** - Inspire and motivate individuals to overcome barriers to recovery. - Conduct strengths/needs assessments and create individualized Service Plans based on client preferences. - Assist clients in accessing and securing necessary services using personal transportation as needed. - Maintain and update Service Plans regularly to reflect client progress and needs. - Build and sustain ongoing relationships with clients and their support systems to ensure effective implementation of treatment plans. - Monitor client participation and progress in services, advocating for their needs with service providers. - Facilitate access to community resources and support networks to promote daily living skills and self-determination. - Coordinate and ensure continuity of care within the human service system. - Respond to emergency situations, providing assessments and referrals as needed, including facilitating commitments to psychiatric facilities. - Maintain accurate and up-to-date case records and prepare required reports within designated time frames. - Adhere to internal and external case management standards, ensuring compliance with all requirements. **Qualifications:** - Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or a related social science; OR - Associate degree with at least 12 college credits in a relevant field AND two years of related experience. - Valid driver’s license and automobile required. - PA Child Abuse, PA Criminal History, and FBI clearances are mandatory. **Application Process:** If you are ready to join a dedicated team and make a significant impact in the lives of others, apply today!
