This role will provide a thorough examination of medication bottles. The Med Rec LPN will review of patient provided lists, computer generated external medication history information, information from other healthcare facilities by contacting pharmacies to obtain and/or clarify medication information and supplements. They will update the medical record to reflect accurate medication history. **Medication Reconciliation LPN Qualifications:** - High School diploma or equivalent required. - Technical Diploma in practical nursing required. - Associate Degree in practical nursing preferred. - Graduated from an accredited practical nursing program required. - Current LPN Licensure with the Arkansas State Board of Nursing. - BLS/CPR required upon hire. - This is a safety sensitive position. \* - ***Minimum Requirements***: - Exhibits flexibility and ability to function under pressure - Displays close attention to detail, ability to handle multiple tasks simultaneously, and the ability to meet deadlines and work with frequent interruptions. - Demonstrates the ability to address and resolve conflict in a professional manner by following the appropriate chain of command - Demonstrates organizational skills while requiring limited supervision - ***Preferred Requirements***: - Previous experience in medication reconciliation, clinical nursing, or other related field preferred. - Proficiency in using electronic health records and other healthcare software - 3+ years of experience working as an LPN in a hospital setting preferred. **Benefits & Perks: your health and happiness matters!
