## Description **POSITION SUMMARY** The **Manager of Care Coordination (RN)** is responsible for the leadership, development, implementation, and oversight of KC CARE's Care Coordination and Nursing Programs. This position provides supervision of Patient Health Coordinators (PHCs), Care Coordinators, and assigned nursing staff while ensuring patients receive comprehensive, patient-centered care that addresses medical, behavioral health, social, and community-based needs. Serving as the operational leader for Population Health nursing and care coordination services, the Manager provides clinical oversight, workflow development, quality monitoring, and staff development. In partnership with the Vice President of Population Health, the Manager supports population health strategies, value-based care initiatives, quality improvement efforts, and compliance with organizational, grant, payer, and regulatory requirements. The Manager also serves as a clinical resource for complex patient needs, fosters interdisciplinary collaboration, and leads initiatives that improve patient access, care transitions, chronic disease management, and overall health outcomes. **KC CARE CULTURE CODE** KC CARE follows a culture code in all we do. As an employee of KC CARE, you will: · Put patients first, always · Treat all people with dignity, respect, and kindness · Create safe places for others to share their voice; encourage creativity · Always strive for improvement; keep learning · Own your work, action, and mistakes – no one is perfect · Have fun – work should be fun and we want you to have fun at KC CARE **** **ESSENTIAL DUTIES AND RESPONSIBILITIES** · Ensures compliance with and supports organizational performance for Patient-Centered Medical Home (PCMH), Primary Care Health Home (PCHH), HRSA, Medicaid, value-based care, grant-funded initiatives, and other applicable federal, state, and payer programs through effective care coordination, care management, quality improvement, and population health activities · Provides direct supervision and leadership of Population Health nursing staff and nursing programs. · Oversees nursing workflows related to care management, chronic disease management, transitions of care, preventive services, and population health initiatives. · Ensures nursing programs operate in accordance with evidence-based practice, organizational policies, and regulatory requirements. · Partners directly with the Vice President of Population Health to establish departmental goals, strategic priorities, quality initiatives, and workforce planning. · Provides coaching, mentoring, and professional development opportunities for nursing staff. · Monitors nursing productivity, quality measures, patient outcomes, and program effectiveness. · Participates in workforce planning, recruitment, retention, and succession planning for nursing and care coordination staff. · Oversees nurse-led care management programs for high-risk, medically complex, and vulnerable patient populations. · Leads nursing initiatives focused on chronic disease management, preventive care, care transitions, and population health outcomes. · Collaborates with clinical leadership to develop protocols and workflows that support team-based care and improve quality performance. · Ensures appropriate coordination between nursing, medical, behavioral health, dental, pharmacy, and community-based services. · Utilizes population health data and quality metrics to identify gaps in care and implement interventions to improve outcomes. · Oversees quality monitoring and performance improvement activities for nursing and care coordination programs. · Tracks and reports key nursing and population health metrics, including care management outcomes, transitions of care, preventive services, and quality measures. · Collaborates with the Vice President of Population Health and Quality Department to develop…
