## Description **POSITION SUMMARY** The **Manager of Care Coordination (RN)** is responsible for the leadership, development, implementation, and oversight of KC CARE's Care Coordination and Nursing Programs. This position provides supervision of Patient Health Coordinators (PHCs), Care Coordinators, and assigned nursing staff while ensuring patients receive comprehensive, patient-centered care that addresses medical, behavioral health, social, and community-based needs. Serving as the operational leader for Population Health nursing and care coordination services, the Manager provides clinical oversight, workflow development, quality monitoring, and staff development. In partnership with the Vice President of Population Health, the Manager supports population health strategies, value-based care initiatives, quality improvement efforts, and compliance with organizational, grant, payer, and regulatory requirements.
