We are looking for a Home Health Licensed Practical Nurse (LPN) to join our team PRN and help our patients remain comfortable, *at home*. This is a great opportunity for a Licensed Practical Nurse with home health experience who loves the life-changing impact your nursing work has on your patients! **You Will:** - Provide nursing care under the direction of the Home Health Registered Nurse and the established Plan of Care to achieve positive patient outcomes. - Communicate with family members and other members of the patient's multi-disciplinary team **What We Need from You:** - A commitment to helping our home health patients remain comfortable, at home. - Proof of graduating from an accredited school of practical nursing. - Current LPN license from the State of Ohio. - Previous experience in home health care setting preferred. - Superior organizational and time management skills. - Reliable transportation, with a valid driver's license, insurance and registration to travel in assigned territories. - Current CPR certification. - Demonstrated compassion, empathy and strong customer service skills. - A completed and successful TB test, physical examination and a criminal background check. **What We Offer to You:** - Your voice matters!
