This position will be traveling to see hospice patients in the Columbia Gorge service area. **Schedule**: Weekdays day shift with rotating weekend coverage The LPN/LVN works under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assists with collection of assessment data and provides input to the planning of care. Implements plan of care within the scope of licensure. As a Providence caregiver, you’ll apply your specialized training to deliver world-class health with human connection and make a difference every day through your extraordinary care. **Required Qualifications:** - Graduate of an accredited LPN/LVN school of nursing. - Within 30 days of hire: National Provider BLS - American Heart Association - Upon hire: Oregon Licensed Practical Nurse - Upon hire: Washington Licensed Practical Nurse - Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
