Role and ResponsibilitiesAdminister practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse.Essential FunctionsUnder the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse: - Provide health education according to the teaching plan established by the RN. - Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN. - Observe signs and symptoms, and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition, to the physician and RN Case Manager. - Document any services rendered to the patient, changes noted in the patient’s condition, and/or family and home situations. - Document communication with RN Case Manager and physician. - Other duties as assigned. Qualifications and Requirements - Current, valid LPN license issued from NYS Department of Education. - Experience in home health care preferred. - Community health experience preferred. Work EnvironmentThe LPN is primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as medium work: - Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
