**Licensed Practical Nurse (LPN) – Home Care** **Location:** West Philadelphia, PA **Schedule:** Contract, 40 hours weekly, 8am – 430pm **Position Overview** The Licensed Practical Nurse delivers high-quality, patient-centered nursing care to clients in their homes. Working independently and as part of an interdisciplinary team, the LPN supports individuals with acute, chronic, or post-operative needs, promoting health, independence, and quality of life. **Key Responsibilities** - Provide direct nursing care in patients' homes in accordance with individualized care plans - Administer medications, injections, and treatments as prescribed - Perform wound care, dressing changes, and monitor healing progress - Assess patient condition and report changes to the RN or care team - Assist with activities of daily living (ADLs) such as bathing, grooming, and mobility support - Educate patients and families on disease management, medications, and home safety - Maintain accurate and timely clinical documentation - Coordinate care with RNs, physicians, and other healthcare professionals - Ensure infection prevention and control practices are followed in the home setting - Travel between patient homes within the community **Qualifications** - Active LPN license in Pennsylvania - Current BLS (Basic Life Support**)** certification - Valid driver's license and reliable vehicle (required for community visits) - Minimum 2 years of nursing experience preferred - Previous home care or community health experience is an asset - Wound care experience Apply today to be part of a dedicated healthcare team! *Prosperity thanks all applicants for their interest in this opportunity.
