Community Health Alliance is looking for a **nurse** to provide care in an outpatient behavioral health clinic at locations in Hamilton, Middletown and Franklin under the direction of a Registered Nurse, Certified Nurse Practitioner or Physician. **The schedule for this position is as follows:** **Tuesday 7am-3pm, Wednesday 5am-1pm, Thursday 8am-4pm, Friday 8am-4pm, Saturday 5am-10am** Develop a career dedicated to connection, compassion, and community. Community Health Alliance is an employer that offers career opportunities with purpose. **At Community Health Alliance our employees are our greatest asset so we offer a full benefit program designed to support you and your family.** We are proud to offer a comprehensive benefits package: - Affordable medical, dental and vision insurance - Company paid Health Reimbursement Arrangement (HRA) - Generous paid time off (PTO) & paid holidays - Retirement plan with company match – up to 6% - Career ladder – opportunities for professional growth - Company paid life insurance - Free Continuing Education CEUs - Paid license renewal - On-the-job training **Job Duties and Responsibilities:** - Complete nursing tasks as directed by a registered nurse or medical provider. - Administers medications to patients when appropriate and maintains accurate records. - Participates as part of an interdisciplinary team coordinating patient care. - Monitors patient appointments and collects health information and vitals for medical provider. - Educates patients on monitoring and managing symptoms, medication and health conditions. - Collects blood, urine and other specimens as needed for laboratory testing. - Administers injections, TB testing, vaccinations as ordered by medical provider. - Completes health and symptomology screenings to aid in patient care. - Assist in scheduling patient appointments as needed. - Coordinates with other medical facilities to obtain medical records for treatment of patients. - Implements appropriate nursing interventions and actions according to established plan of care within scope of practice. - Administer basic first aid, when necessary. - Accurately and timely documents patient information and interventions in the electronic health record. - Assist with nursing coverage at multiple clinic sites. - Practice within the scope of practice as outlined by the Ohio Board of Nursing. - Licensed in the State of Ohio as a Licensed Practical Nurse (LPN). - Two years related experience. - Current BLS/CPR or willing to obtain within 30 days of hire. - Valid driver's license and reliable transportation (travel between sites required). - Ability to work flexible hours, including early morning, weekends or evenings if needed. - Experience in substance use treatment, behavioral health or related nursing field preferred. - Knowledge of medication-assisted treatment (MAT), harm reduction and trauma-informed care is preferred. - Ability to collaborate with all members of the healthcare team. - Commitment to providing nonjudgmental, empathetic care to individuals with substance use and behavioral health disorders. - Effective verbal and written communication skills. - Exceptional customer service skills. - Must be able to work as a team member. - Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams. - Adept at learning and navigating electronic health record systems. **Community Health Alliance** is a leader providing the highest industry standards for health and human services.
