## Description **POSITION SUMMARY** The Behavioral Health Licensed Practical Nurse (LPN) is a member of the CIC team. This individual will use the nursing process, which involves assessment, outcome identification, planning, implementation, and evaluation, as the foundation for clinical decision making for all person’s served. The Behavioral Health Registered Nurse works with a multi-disciplinary team to provide and coordinate optimal services for individuals experiencing acute behavioral health crisis. This position requires the ability to be on-call and rotation of holidays. If there are periods when your primary responsibilities are limited, you may be asked to assist in other areas as needed to support overall operations, regardless of your specific role. This position is essential to operations and must be staffed at all times. Therefore, the current employee in this role will need to remain in place until a replacement is able to assume responsibilities. **ESSENTIAL FUNCTIONS** - Assessment: The Behavioral Health Nurse collects health data, including vital signs and behavioral health changes, and documents in the electronic health record (EHR). - Outcome Identification: The Behavioral Health Nurse identifies expected outcome individualized for the person served. - Planning: The Behavioral Health Nurse develops a treatment plan that identifies interventions to attain expected outcomes. - Implementation: The Behavioral Health Nurse implements the interventions identified in the treatment plan. - Promotes the therapeutic relationship and uses counseling interventions to assist persons served in improving or regaining their previous coping abilities, fostering mental health, and preventing the relapse of mental illness and disability. - - provides structure and maintains a safe therapeutic environment in collaboration with the person served, their family/support persons, and other health care providers. - structures interventions around the person served activities of daily living to foster self-care and mental and physical well-being. - uses knowledge of psychobiological intervention, applies clinical skills to restore the person served health, and prevent further disability. - through health education, assists person served in achieving satisfying, productive, and healthy patterns of living. - participates in the coordination of comprehensive health services to promote continuity of care. - employs strategies and interventions to promote and maintain mental health and prevent relapse of mental illness. - will utilize and contribute to clinical supervision process to strengthen and maintain practice outcomes. - Critically evaluates existing practice in the light of current research findings. - works as part of an integrated treatment team collaborating with the person served, family members or others they want involved in their treatment and other treatment team members (internal and external) to help support the person served in their treatment and recovery - Develop a therapeutic relationship with persons served and families promoting decision making based on the assessment of medical histories, symptoms, examinations, diagnosis, conditions and treatments. - Assesses and coordinates with person’s served discharge planning needs with members of the healthcare team. - Utilizing information, resources and technology - Work under the direction of an RN. - Provide support to direct care staff - Administer Medications, medical procedures/treatment as prescribed. - Conduct point of care testing, including vital signs, urine drug screen, and breathalyzer test - Demonstrate cultural and linguistic competence - Approach interactions with persons served and colleagues with a trauma sensitive lens - Provide services that are recovery oriented and person and family centered, utilizing a strengths-based approach - Provide services to meet the whole health needs of person served - Utilize appropriate assessment instruments to help moni…
