Licensed Vocational Nurse (LVN) -Home Health

We are a passionate group dedicated to delivering the highest quality of care to our patients, supporting families, empowering our team members, and enriching the community. **LVN or LPN (Home Health)** ***Service Areas for Coverage: Northwest (PRN or FT), Northeast (FT)*** **Responsibilities:** - Collaborate with the home healthcare team to develop and implement individualized care plans. - Complete home care nursing visits. - Prepare timely, complete, and accurate clinical notes. - Communicate with healthcare providers and other care team members. - Educate patients and their families about health conditions, treatment plans, and self-care techniques. - Adhere to all agency policies, procedures, and regulatory requirements related to home health care. **Qualifications:** - Valid and current LVN or LPN license in the state of Texas or compact state. - Minimum of one (1) year of experience in a home health setting. - Bilingual (Fluent in Spanish) encouraged to apply - Knowledge of home health regulations and general healthcare compliance. - Experience with HCHB EMR.
