We’re building our team in **Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.** ## The mission of this role **Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.** ## Objectives 1. **Carry a full, high-quality caseload:** ramp to **~30 productivity points/week** at target visit quality. 2. **Own your patients' outcomes:** drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. **Be a referral-source ambassador:** every discharge planner, ALF, and physician you touch should *want* to send the next patient to us because you were responsive and excellent. 4. **Set the standard others copy:** your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. ## What you'll own - Deliver skilled nursing visits in the home **within LVN scope**, under RN/clinical supervision as required. - Perform focused assessments, monitor for changes in condition, and **escalate promptly** to the RN/MD/clinical team using clear, objective information. - Support medication reconciliation and medication safety education (within scope), and identify discrepancies or risks for RN review. - Provide patient and caregiver education (teach-back) aligned to the plan of care. - Support wound care and other skilled tasks per orders and competency (e.g., dressing changes, basic monitoring, reinforcement of teaching). - Coordinate with the office and interdisciplinary team so visits happen on time and orders are followed. - Document thoroughly and on time in **Homecare Homebase (HCHB)** — defensible, efficient, and consistent. ## **Who you are / how you operate** - **Dedicated clinician:** home health experience strongly preferred - **High ownership:** you treat your caseload like your own panel and your pod like your own business. - **Open mindset:** Energized by the opportunity to shape the playbook and refine SOPs - **Tech-forward:** You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better - **Trust-builder:** Patients, families, and referral partners believe you because you do what you say. ## ## **Must-haves** - **Active Michigan LVN license (unrestricted)** - Comfort working in a field-based workflow with appropriate escalation judgment - Strong communication and documentation - Reliable follow-through on orders, visit cadence, and patient goals - Valid driver’s license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory ## **Nice-to-haves** - Prior **home health** or hospice experience - Familiarity with **Homecare Homebase (HCHB)** or similar EMR - Wound care exposure ## The rest of the package - **Full-time target: ~30 productivity points/week**; clinicians earn **more** for exceeding it. - **Benefits** for full-time (medical/dental/vision, PTO) - Mileage Reimbursement
