Performs a variety of nursing duties associated with monitoring client’s health and general wellbeing, managing health issues and providing health care.​ Report to nurse manager. **Essential Job Functions** - Assesses and evaluates clients; obtains and reviews pertinent medical data; collects vital signs and specimens; monitors client’s response to treatment. - Follows through with treatment plans while assisting RNs or physicians in providing direct care to patients - Assists with psychological reviews; schedules client appointments with health care providers and dentists. Attends all neurology/specialty appointments, as appropriate - Documents the administration of care in the client medical record in a timely and thorough manner - Provides the medical perspective into client care for all Multidisciplinary Team (MDT) meetings. - Administers medication as directed - Serves as a resource to department staff and students and to help deliver quality client care - Provides crisis intervention services as necessary. - Provides hepatitis B or flu vaccines and PPD testing to Grafton staff. - Promotes and assists in the control of communicable disease. - Prepares and/or completes various health services forms like medication permission forms, on-call sheets, medical/dentist appointment sheets, etc. - Works on-call duty in rotation, as appropriate; promptly responds to calls for immediate nursing services. - Attends meetings, seminars and training as required. - Performs other work as assigned. **Minimum Qualifications** - Requires a high school diploma or equivalent. Associates degree preferred. - Valid driver's license - Valid LPN License from the State Board of Nursing - 1 year or moderate experience as a practical nurse in a behavioral health setting, or equivalent combination of education and experience; previous medication administration - Thorough knowledge of practical nursing techniques - Computer proficiency, strong communication skills, ability to maintain effective working relationships with clients and other employees. **COMPENSATION** position may be filled as LPN or RN - *RN starting at $38.46 up to $51.39/hour; commensurate with experience and qualifications*. - *LPN starting at $30 up to $41.23/hour; commensurate with experience and qualifications.* **EMPLOYEE BENEFITS:** **EMPLOYEE BENEFITS (prn / part-time)** - 401(k), including an **employer match** up to $2,500 annually - Education Assistance up to **$5,250 annually** (must work an average of 20 hours a week) - Employee Assistance Program (EAP) - Five FREE sessions per year, per family member - 24-hour emergency phone assistance - Referral Bonus Program – Employees receive **$1,000** for referring new employees to Grafton ($500 paid upon hire and $500 paid after 90 days of employment) *If you require a reasonable accommodation or need assistance with the application or interview process please contact us at hr@grafton.org* *Grafton is an equal employment opportunity employer and tobacco-free workplace* *#LP*
