Schedule: Monday-Friday, 8:00am-5:00pm, with on day shift of on call per month on a Saturday or Sunday. (The work week is adjusted to allow a day off when working the weekend shift.) Location: Grant, Kenton, Boone counties primarily. At times may need to assist Carroll, Gallatin, or Campbell. Position Summary: The Licensed Practical Nurse (LPN) provides skilled and compassionate nursing care to hospice patients in their place of residence, including private homes, nursing homes, assisted living facilities, or other settings the patient calls home. Working under the direction and supervision of a Registered Nurse, the LPN delivers individualized care in accordance with the established plan of care to promote patient comfort, safety, and quality of life at the end of life. The LPN is responsible for performing assigned nursing and personal care tasks, monitoring and reporting changes in patient condition, educating patients and caregivers, and documenting care accurately and timely within the electronic medical record (EMR). Position Qualifications: • Graduate of an accredited practical nurse or vocational nursing program. • Current LPN licensure in good standing with the Kentucky Board of Nursing. • Minimum of two (2) years of nursing experience; community health/hospice, medical-surgical, or long-term care experience preferred. • Demonstrates compliance with accepted professional nursing standards and practices. • Understands and supports the philosophy of hospice care and the needs of terminally ill patients. • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
