Join our team today. **What You'll Do** - Complete psychosocial assessments of new patients and develop updates or detailed reassessments of the patient. - Maintain contact with family and significant others when appropriate, and provide patient and family education, support, and advocacy. - Provide coordination and act as a liaison with community-based social and mental health agencies. - Facilitate discharge planning and conduct post-discharge follow-up to ensure patients are linked with community resources, reducing re-hospitalization. - Facilitate therapeutic groups with patients daily. **What We Offer** - Service excellence & a fulfilling work environment - 401(k) plan with company match - Health Insurance - Dental Insurance - Vision Insurance - Life Insurance - Paid Time Off - Supplemental Insurance Plans Available **What You'll Bring** **EDUCATION & LICENSURE** Knowledge acquired through completion in social work (SW) from an accredited school of social work. Unrestricted licensure in social work required. LMSW, MSW, or LPC preferred. Will consider CSW or PLPC with stipulations. **EXPERIENCE** At least one (1) year of related experience in a hospital setting, with psychiatric experience preferred. **ADDITIONAL REQUIREMENTS** Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment. **SCHEDULE** Seeking Full-Time with some weekends. **About Perimeter Healthcare** Perimeter Healthcare is one of the leading mental and behavioral health care providers in the country.
