**Scheduled Weekly Hours:** 40 **Schedule**: Thursday, Friday, Saturday, Sunday, & Monday (7:00am-3:00pm) **Pay Range Minimum:** $25.00 **Education We're Looking for:** Associates (Required), Bachelors **Certifications We're Looking For:** Drivers License (Valid) - USA **Experience We're Looking For:** Experience working with adults in a behavioral health capacity | 2-3 Years (Dependent on Education) | Required **Additional Details:** **The Lead Mental Health Worker, RI must have one of the following:** - **Associate's degree with 3 years of experience working with adults in a behavioral health capacity** - **Bachelor's degree with 2 years of experience working with adults in a behavioral health capacity** **Job Description Summary:** The Lead Mental Health Worker provides person-centered support, care coordination, and oversight of daily program operations to promote positive outcomes for individuals receiving services. This role is responsible for assessing needs, coordinating and monitoring service plans, ensuring accurate documentation, and maintaining compliance with agency, state, and federal requirements. The Lead Mental Health Worker also provides guidance, training, and oversight to Mental Health Workers, ensuring staff understand treatment plans, complete required responsibilities, and deliver high-quality services. Additionally, the role supports a safe and therapeutic environment through crisis response, incident management, case management, care coordination and collaboration with individuals served, families, and interdisciplinary teams. **What You'll Do:** - Provide initial and ongoing assessment, evaluate effectiveness of person-centered plan and progress made by person receiving services, participate in team meetings with other providers and natural supports as needed and facilitate development of person-centered plan. - Identify methods whereby services can be provided using existing community resources whenever possible. - Continuously use person-centered approaches and positive approach strategies when interacting with people we support. Integrate strategies into program plans. - Ensure requests for services are made considering eligibility, continued stay and discharge criteria for the service definition. Services and support that match the person-centered plan and the individual’s needs shall be in place and be fiscally viable. - Complete any case management needs for admissions related to benefits, insurance, and referrals. Also, ongoing case management for medical and psychiatric functioning and stability. - Ensure staff are appropriately trained regarding plans and related programs and demonstrate an understanding of specific plan components (including but not limited to all privileging requirements as outlines by area programs). - Ensure that all required and/or requested reports/documentation are completed. Maintain the records of the individuals supported. Coordinate the completion of daily, monthly, quarterly, and annual documentation in the EHR and/or medical record. - Use a range of communication skills and strategies to establish a mutual relationship with the individual, staff, co-workers, supervisors, other stakeholders and people who are important to the individual’s receiving services. - Respond effectively and timely to all incidents following agency policies and procedures. - Maintain a safe working environment for employees and people receiving services. - Demonstrate knowledge of emergency procedures including preventative and crisis services. Schedule and participate in on-call coverage.
