We are looking for someone with experience working with a wide range of patient injuries and age groups, who is also ready to learn new techniques to further patient care. The person hired for the Physical Therapist position would create a plan of treatment consisting of the appropriate exercises, procedures and techniques required to promote effective, safe recovery. They will also be coordinating with any healthcare professionals that are working with their patients and referring patients to specialists when appropriate. The right candidate is a patient, compassionate professional who encourages and inspires their patients to work toward greater independence and recovery. **Essential Job Functions:** 1. Work with supportive personnel, assessing their competence, delegating specific tasks to them and establishing channels of communication. 2. Restores patient’s function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy. 3. Provide educational information about physical therapy and physical therapists, injury prevention, ergonomics and ways to promote health. 4. Provides quality care by assessing and interpreting evaluations and test results. 5. Plan, prepare and carry out individual designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients. 6. Helps patient accomplish treatment plan by administering manual exercises, giving massages, and using equipment as necessary to help relieve pain, increase the patient’s strength and decrease or prevent deformity and crippling. 7. Evaluates and records patient’s progress, modifying treatment plans, and trying new treatments if required. 8. Evaluates the use and fit of prosthetic and orthotic devices by observing, noting, and observing patient’s progress; and recommending adjustments and modifications to achieve maximum benefit. 9. Review physician’s referral and patient’s medical records to help determine diagnosis and physical therapy treatment required. 10. Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals. 11. Documents patient care services by charting in-patient and department records. 12. Instructs patients in proper in-home exercises and therapies. 13. Direct group rehabilitation activities. 14. Educates patients and their families about the recovery process and the challenges they will face. 15. Works with Yellowhawk providers to promote the overall health and wellness of the patients we serve. 16. Other duties as assigned ## Requirements **Minimum Qualifications:** 1. Current, valid and unrestricted Physical Therapy license and board certification. 2. 5 years successful experience as a Physical Therapist. 3. Current Basic Life Support (BLS) certification with AED. 4. Demonstrated experience administering a wide range of physical therapy techniques not limited to: massage, mobilization, manipulation, and strengthening exercise; hot and cold therapy; ultrasound, traction, laser therapy and electrical stimulation. 5.
