This is an exciting opportunity for someone with a background in social care who is passionate about making a real and lasting difference to the lives of older people using our integrated Intermediate Care service. **The shifts for this role are 4 x 3 hour shifts per week 19.45pm - 23.45pm.** **About the Role** As an Intermediate Care Assistant, you will play a key role in supporting older people to regain independence, improve wellbeing and achieve their personal outcomes.
