Intensive Case Manager - Post-Release Services L3

**Classification:** Exempt **Status:** Full Time; Flex schedule (employee must reside in Kentucky) **Salary:** $63,000 annually **This is a salaried position - billable units are not required.** **About Home Study and Post Release Services** KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky and potentially contiguous states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to: - Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study). - Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and linking children to resources in the community (Post Release Services). **The PRS Intensive Case Manager (ICM)** provides intensive case management services for families needing additional support. Their primary focus is stabilizing the family unit through tailored interventions and connecting the released child and sponsor family to community-based services for long-term care. PRS ICMs deliver Level 3 Post Release Services and may also provide Level 2 and Level 1 services. This role requires flexibility to work outside standard business hours, including evenings and weekends, as the HS/PRS program operates on a 24/7 schedule. Team members coordinate schedules to meet client needs, with approval from the Program Director or PRS Case Management Supervisor. **Education and Experience Requirements** - Master’s degree in social work, psychology, sociology, or relevant behavioral science in which direct clinical experience is a program requirement **OR** Bachelor’s degree with a minimum of 5 years of clinical experience - May be licensed or eligible for licensure (i.e., CSW/LCSW, LPCA/LPCC, AMFT/LMFT) - Highly fluent in English and Spanish (oral and written) - Knowledge of local community social services - Specialized experience working with immigrant populations - Knowledge of family preservation, kinship care and child welfare * * * **Benefits Include:** - Recruitment incentive of $3,000 available during first year of employment - Free Employee Assistance Program - Health, vision, dental, and life insurance; - 403b retirement plan with 3% match - Paid time off for sick time, vacation, and mental health care - Eight paid holidays - Tuition assistance and continuing education support **Essential Functions and Competencies** - Deliver comprehensive mental health services, including psychoeducational support for recently migrated youth. - Collaborate with youth, families, and case management team to develop short-term goals and treatment plans. - Conduct weekly in-home visits for at least 30 days and maintain monthly contact for a minimum of 6 months. - Provide in-person support during stressful or traumatic experiences (e.g., medical appointments, hospital visits). - Document all client contacts, interventions, and assessments in compliance with KYUMH and ORR standards. - Develop culturally competent clinical interventions to address trauma and assist youth in adjusting to life in the U.S. - Conduct screenings for mental health, developmental needs, trauma history, and child trafficking; provide referrals as needed. - Provide individual, family, and group therapy, crisis intervention, and psychoeducation. - Maintain effective working relationships with families, colleagues, and community agencies. - Participate in case review meetings and present accurate client summaries. - Ensure compliance with documentation standards in agency, state, and federal databases. - Demonstrate cultural humility and trauma-informed practices. - Requires frequent travel; occasional out-of-area and overnight travel, including airline travel - Competencies include: - Accountability and reliability - Empathy and emotional control - Flexibility and stress management - Cultural awareness and sensitivity - Clear communicatio…
