Position Summary As an Intake Psychiatric Mental Health Nurse Practitioner at Blackbird Health, you'll play an essential role in delivering comprehensive psychiatric intake evaluations for children, adolescents, and young adults. You'll build meaningful connections with patients and families from the very start of their care journey, spending the necessary time to understand their needs, educate them about care options, and ensure a seamless transition into ongoing treatment. Working alongside an interdisciplinary team, you'll be empowered to make a tangible difference in the lives of young patients and their families from day one. This remote role requires a minimum of 10 hours per week. How You'll Make an Impact Deliver Comprehensive Intake Evaluations: Conduct thorough psychiatric intake evaluations for children, adolescents, and young adults. Develop individualized treatment plans that address the unique needs of each patient and establish a strong clinical foundation for their ongoing care. Drive Strong Clinical Matches: Assess each patient thoroughly and collaborate with the clinical team to ensure every patient is connected to the right ongoing provider and treatment approach. Collaborate for Integrated Care: Work closely with therapists and other healthcare professionals to create coordinated treatment plans that address the whole child. Communicate and engage with families during the intake process to ensure a warm, supported entry into care. Educate & Empower Families: Provide education and consultation to patients and their families during the intake process, helping them understand diagnoses, treatment options, and how to support their child's mental health journey.
