A Home Health Intake Coordinator manages the initial patient referral process, acting as the key liaison between hospitals, physicians, families, and clinical staff to ensure a seamless start of care. They are responsible for receiving referrals, verifying insurance benefits, obtaining authorizations, and accurately entering patient data into Electronic Medical Records (EMR). As a Intake Referral Coordinator, you will: - Receives and appropriately documents new referrals in a timely manner. - Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications. - Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group. - Provides effective communication to patients, staff members, other health care professionals and referral sources. - Responds to Agency and patient needs in a professional and creative manner. - Demonstrates commitment, professional growth and competency. - Participates in Agency-sponsored in-service training. - Participates in the Agency’s QAPI program. - Promotes the Agency philosophy and administrative policies to ensure quality of care.
